Kindergarten Enrollment & Screening Information
Click the link below to fill out the Kindergarten Enrollment Form online. All new Kindergarten students must have a completed enrollment form before attending Kindergarten Screening.
Students who do not live in the Davenport A+ district must also have a Transfer Application completed. Families who live out-of-district will have to complete this form EACH YEAR in order for their student(s) to continue to attend Davenport A+. If you live in the Davenport A+ district, you do not need to fill out the Transfer Application.
